Initial Visit: $395

Single: $99/mo

Couple: $175/mo

 New patients are required to pay a $100 deposit at the time of booking. The deposit will be applied toward the $395 new patient visit. 

Deposits are non-refundable.

 If the new patient appointment is rescheduled or canceled within 24 hours of the visit, a new $100 deposit will be required to secure a future appointment.

• One 45-min wellness follow-up every 3 months (4/year)
• Direct portal messaging
• 15% off supplements 
• Free functional medicine interpretations
• Member visit pricing: $125 for additional visits (>2 in 6 months)
• Access to discounted labs & compounded medications
• Access to at-home curriculum that provides education & accountability to support sustainable lifestyle changes between visits

*Memberships for adults 18 years and older. 

Patients may utilize their health insurance for laboratory testing when desired and clinically indicated. 

Cash-pay laboratory options are also available. 

Membership pricing does not include the cost of laboratory tests.
